Kentucky Mountain Health Alliance, Inc. Transportation Agreement

Operating Hours: Monday through Friday (7:30 AM to 3:30 PM)

1. Transportation will be provided for active patients of the clinic unless the administration has made other arrangements.
2. For personal safety, we require all patients and caregivers to ride in the back seat during all transports. (NO ONE IS PERMITTED TO RIDE IN FRONT SEAT UNLESS EMPLOYED BY KYMHA)
3. The Kentucky Seat Belt Law requires that every driver and passenger wears proper safety belts when the vehicle is in motion.
4. Only the patient with an appointment will be transported unless otherwise directed by the administration.
5. If our clinic is transporting a child, the parent or guardian is required to provide proper safety seating for the child.
6. All children under the age of 18 should be accompanied by a parent or guardian when being transported. (If accompanied by a guardian, guardianship papers must be present.)
7. In the calendar month, patients are allowed 2 nonmedical transports per household. Examples of nonmedical would be grocery stores, the Social Security Office, banks, the local DCBS office, New Hope, etc.
8. All medical transports must be scheduled 24 hours before being transported; all nonmedical transports must be made 48 hours in advance. Appointments may be adjusted accordingly.
9. Transporters may arrive 1 hour before the scheduled pick-up time. Transporters will wait no longer than 15 minutes when arriving at the location for pickup.
10. Patients will be picked up at the same location where they have been dropped off. Example: If you are dropped off at the ARH Medical Mall, you are to be picked up at the ARH Medical Mall.
11. No-Show is transport that has not been canceled, or a transporter has gone to your home, and/or called with no answer. If a patient has 2 No-Shows, it’s the patient’s responsibility to get to their next scheduled appointment at our clinic with their own transportation. When the patient keeps the next appointment, transportation can be re-established.
12. All transportation appointments can be made by any KMHA personnel. If you need to cancel a pickup, please do so 2 hours before your appointment. If you have any transportation issues, please contact our office today at (606) 487-9505.
13. Patients are not permitted to carry any weaponry, which includes but is not limited to knives, sharp objects, and any sort of guns while being transported by KMHA. We reserve the right to determine if an object can be deemed a weapon.
14. Transporters should have proper identification while transporting patients.
15. Transporters may not give or receive any money or any other items, so please be courteous and do not ask.
16. Transports can only be done in company vehicles.
17. Transporters cannot take you to the emergency room. For emergencies, please dial 911.

Kentucky Mountain Health Alliance, Inc. Reserves the right to amend or make changes to the terms of this notice and to make the revised or changed notice effective for information we already have about you as well as any information we receive in the future.
